The MBTA operated four shelter bus routes that were not open to the general public: the 274, 275, 276 and 277. With the closure of the Long Island Shelter on October 8, 2014, the last special hospital bus route that remains is the 277.

The 277 is a rather elusive bus route indeed. It departs daily from Lemuel Shattuck Hospital in Jamaica Plain at 8:00 AM. From there, it travels along the route of the 16 to get to Blue Hill Avenue. Then it travels along the route of the 22 on Seaver Street and Columbus Avenue via Egleston Square, Jackson Square and Roxbury Crossing up to Ruggles (but it does not go into the station). Finally, it travels along the route of the 43 along Tremont Street to get to Park Street.

The 277 operates out of the Quincy Garage. Up until late 2012, RTS buses were always used on the 277. This was due to weight restrictions on the Long Island Bridge, even though the 277 did not go to Long Island. Now New Flyer buses do all the trips.

I believe anybody can ride on the 277, as it does not enter into any restricted areas. However, the only intermediate stop this bus route makes is Tremont Street at West Springfield Street (the closest stop to Boston Medical Center on Tremont Street).
The 277 can be considered a very rare sight to see, unless you are always around Park Street around 8:30 in the morning.
As of Fall 2015, the 277 has been renumbered 195 and operates from Arborway.
